
South West Family Medicine Associates (SWFMA), a medical group practice located in Dallas specializing in Family Medicine and Nursing, has alerted 36,959 patients and employees that their personal and health information may have been exposed following a cybersecurity breach identified in August.
Suspicious activity was detected within SWFMA’s network on August 7, 2024, prompting an investigation by cybersecurity experts. It was later confirmed that unauthorized individuals had accessed or potentially acquired sensitive data files.
The review of compromised files, completed on October 24, revealed that a broad range of patient and employee information was impacted. The exposed data includes names, addresses, Social Security numbers, driver’s license details, dates of birth, medical histories, and lab results, among other sensitive identifiers. Additionally, some records contained information such as health insurance details, tax identification numbers, and even personal credentials used for financial or electronic authentication.
While no data misuse has been reported, SWFMA urged affected individuals to remain vigilant. The organization offers complimentary identity theft protection and credit monitoring services, including dark web monitoring and a $1 million insurance policy covering identity theft. These services aim to mitigate potential risks from the breach, offering support and resources for those affected.
